As part of our Community Health Needs Assessment, 1st Choice Healthcare is holding community forums to gather feedback from those we serve.
To protect the health and well-being of community members, the community forums will be held virtually. We have scheduled community forums over three dates to provide opportunities for participants to attend.
Each forum will be scheduled for one hour.
September 15 (9 a.m.-10 a.m.)
September 22 (4 p.m.-5 p.m.)
September 29 (12 p.m.-1 p.m.)
Please complete the short survey linked below. The information shared in this short 10-minute questionnaire will serve as a key input for 1st Choice’s Community Health Needs Assessment. Please share honestly and openly as your responses will remain anonymous.

Discussion and Agenda Items
- Greatest Healthcare Needs
What are some of the greatest healthcare needs that low-income persons face in our community?
- Top 5 Needs
Of the needs discussed, what are the top 5 needs of our community?
- Community Focus
What top three improvements do you think 1st Choice Healthcare or the community should focus on?

- Assets, Resources and Solutions
We will seek to identify community assets and resources that can address the needs and identify solutions. What suggestions can you provide on how the needs could be addressed?
