1st Choice Community Forum — Community Needs Assessment

As part of our Community Health Needs Assessment, 1st Choice Healthcare is holding community forums to gather feedback from those we serve.
To protect the health and well-being of community members, the community forums will be held virtually. We have scheduled community forums over three dates to provide opportunities for participants to attend.
Each forum will be scheduled for one hour.

What top three improvements do you think 1st Choice Healthcare or the community should focus on? - Identifying Contributing Factors
For each healthcare need, we will seek to identify the conditions, root causes and barriers contributing to the need. What do you think are some of the gaps or barriers to health services in the community and do you have any suggestions related to this? - Assets, Resources and Solutions
We will seek to identify community assets and resources that can address the needs and identify solutions. What suggestions can you provide on how the needs could be addressed? - Do you have any other feedback?

As part of our Community Health Needs Assessment, 1st Choice Healthcare has hired Wipfli, LLP a nationally recognized consulting firm, to serve as moderator for our community forums.
