Please make sure to bring a list of all medications and a copy of your insurance card to each visit. If there have been any changes to your personal information or if it has been a year or more since you have been seen, you may be asked to fill out a new registration form.
New Patients
New patients should arrive at least 30 minutes early to complete registration paperwork.Cancellations
We call to auto-confirm your appointment two business days prior to your planned visit. In the event that you must reschedule an appointment, please notify our office at least 24 hours in advance to avoid a cancellation fee.After Hours & Emergencies
If you have an urgent matter after hours and need to speak to the physician on call, please dial the clinic number and follow the prompts to speak to a nurse. If you experience a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY.A medical release form must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
Prescription refills are best handled at your regularly scheduled office visit. During this time, we are able to review the proper dosages, as well as address any questions or concerns you may have. If it becomes necessary to refill your medications between appointments, please allow at least 24 hours for prescriptions to be phoned to pharmacies and at least 48 hours to process requests for written prescriptions.
Chronic or long-term medication refills require an office visit before they may be refilled.
We accept: Medicare, Medicaid, Private Insurance Plans, Cash or Check, Credit Cards, and Sliding fee/discount payments.
We ask that patients pay for services at the time services are rendered. Proof of income is required for discounted fees. This service is available to individuals and families who wish to apply and who qualify. Patients receiving the discount pay minimal fees.
Chronic pain – pain lasting more than six (6) months – is a growing problem in our country. As our population ages, more and more patients are experiencing chronic pain. Chronic pain is difficult to treat because treatment often consists of highly addictive medications. These medications – such as hydrocodone, oxycodone, or morphine – are so addictive that they are frequently sold, traded, or stolen instead of being used for their original purpose.
Arkansas is facing increasing problems with medication misuse, addiction and overdose. Because of these problems, regulatory agencies are increasing their surveillance of physicians and other licensed providers who prescribe these medications. In the state of Arkansas, a physician or provider who over-prescribes these medications or inappropriately prescribes these medications can lose his or her license, thereby ending that physicians or provider’s career.
To ensure the safety of 1st Choice Healthcare patients, and to ensure that our organization is in compliance with state recommendations and rules, we have established additional guidelines. 1st Choice Healthcare currently has a “Medication Agreement & Refill Policy” that all patients are required to sign. This agreement addresses many issues regarding narcotic medications, including the additional guidelines. If you have misplaced your agreement, or would like another copy, please notify the front office staff.

What is Patient Centered Medical Home?
Patient Centered Medical Home (PCMH) is a partnership between you as a patient and your primary care physician. You are the center of the team which is lead by the physician. The goal is to provide continuous coordinated healthcare throughout your lifetime so that you will be as healthy as you possibly can be.
Why should you have a medical home?
PCMH is a way for you to be informed about and involved in your healthcare decisions. The PCMH can bring you, your family and your healthcare team together to help you make the best choices about your health.
A Medical Home is an evidence–based approach to provided total health care. At the heart of the PCMH is the relationship between you, the patient, and your healthcare team led by your doctor or provider of choice. Your healthcare team will include health care professionals, trusted friends or family (if you wish), and – most importantly – you.
Good Faith Estimate
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
